In many cases you will not need DS4w for the controller to work properly but hold on to the program for the future. It's best if your has a USB 3.0 port vs a 2.0 I tested many times to see if this mattered or not. What's more is your controller might only disconnect while playing For Honor or other Ubisoft titles! After months of trouble shooting I have found the answer to this problem! And in a few cases even the Xbox controllers would randomly disconnect.
